Dear Parent:

Cook Children’s needs your help with the following confidential survey about children’s health in our area. We regularly conduct a Community Health Needs Assessment (CHNA) to identify the children’s health needs in our community and develop solutions with community partners to address the most important issues. We need information from parents of children ages 0-17 in the following counties: Collin, Denton, Grayson, Johnson, Hood, Parker, Tarrant, and Wise counties.

We will add your responses to this private survey to responses received from other parents. With information from parents throughout the region, we can identify the strengths and challenges to make our community the healthiest place to raise children.

We invite you to take a few minutes to complete this private survey in the next few days. It is a simple way that you can give back to your community and make our region an even better place to live. Your participation is needed even if you have never used Cook Children’s services.

ETC Institute is a research firm that works with communities all across the country. ETC will compile the survey results for Cook Children’s and we will provide online access to the results in the coming months.
